As SANICA Testing Laboratory, we perform tests and analyses of plastic pipes and fittings. Please fill out the form below completely to request a testing service. Your request will be evaluated and a quote will be prepared as soon as possible.

* For the tests to be applied section, it is sufficient to write the test sequence numbers. Sequence numbers are specified in the Test Services List.

General Terms

  1. Samples suitable for the requested test will be delivered to the laboratory by the customer. Transportation and preservation of samples to the laboratory are the responsibility of the customer.
  2. The period for objections to test results is one month from the date of result notification. Objections must be made in writing to the Quality Manager. Tested samples and witness samples not collected within this period will be disposed of by SANICA.
  3. Explanatory documents related to the sample (excluding standards) will be provided by the customer to the laboratory.
  4. The accredited test scope can be accessed at www.turkak.org.tr.
  5. Tests will commence after the approved version of this form reaches us and the test fee has been paid.
  6. In case of any deviation from the agreed terms, the customer will be informed in writing or verbally.
  7. If the customer does not send a witness sample together with the original samples at the time of request, the customer accepts and undertakes that they will not object to the test results or request a repeat test.
  8. The customer or their representative may be present in the laboratory during the testing of their own sample.
  9. When samples are sent by cargo, no "Sample Receipt Form" will be issued.

  11. All material and moral liabilities arising from the notification of test results to the "Report Delivery Address" specified above belong to the customer.
  12. This Test Request and Quotation Form becomes a contract when approved by both parties. Customers may, when necessary, submit objections to test results or make suggestions by filling out the complaint/suggestion form at sanicaboru.com.tr. The evaluation result will be communicated as soon as possible. If the dispute continues, the test(s) will be repeated at a third-party Accredited Laboratory and a final decision will be made.
  13. When making conformity assessment decisions, SANICA does not pursue commercial concerns and applies the decision rule as specified in article 14, in line with public interest, to ensure the protection of the public from environmental hazards.
  14. When performing conformity assessment, uncertainty results will not be taken into account; in case a different conformity assessment is required, the laboratory must be contacted and the operation of the FR71 Decision Rule Agreement Form will be requested. If no such request is made, uncertainty results will not be considered during evaluation.
  15. When the laboratory is required by law or contractually authorized to disclose confidential information, unless prohibited by law, the customer or relevant person is deemed to be informed about the information to be disclosed.
